Item name, an optional recipe label used for matching, and up to 10 price / unit conversions (e.g. case, lb, oz).
One row per price. RECIPE LABEL and VENDOR are optional -- leave them blank. Give the same item name on multiple rows to record multiple price/unit combinations (e.g. a case price and a per-pound price, or the same unit from two different vendors).

This is close to what a spreadsheet (Excel, Google Sheets) or another inventory system's price-list export already looks like -- reorder/rename the columns to match and it should import cleanly. For an exact backup of everything in your account (cost guide, recipes, inventory, sales log, and settings together), use Backup & Restore instead.
